The Alarming Statistics on LGBTQ+ Substance Use
Decades of research have revealed a troubling disparity in substance use rates between LGBTQ+ individuals and the general population. According to the Substance Abuse and Mental Health Services Administration (SAMHSA):
LGBTQ+ adults are more than twice as likely to use illicit drugs than heterosexual adults (SAMHSA, 2020).
1 in 5 LGBTQ+ adults struggles with a substance use disorder, compared to about 1 in 10 heterosexual adults (NIDA, 2023).
Transgender individuals report especially high rates of substance use, often tied to experiences of discrimination, trauma, and lack of access to affirming care.
In addition, a 2021 report from the Trevor Project found that over 60% of LGBTQ+ youth reported symptoms of depression, and nearly half had seriously considered suicide in the past year (Trevor Project, 2021). Mental health issues are closely intertwined with substance use, especially when left untreated.
Why Is Substance Use So Prevalent in the LGBTQ+ Community?
Substance use doesn’t happen in a vacuum. For many LGBTQ+ individuals, drugs or alcohol become a way to cope with:
Social stigma and discrimination: Repeated exposure to prejudice—from family, workplaces, healthcare providers, or society at large—can create chronic stress, often called minority stress.
Family rejection: According to the Human Rights Campaign, LGBTQ+ youth who experience family rejection are 8 times more likely to attempt suicide and 3 times more likely to use illegal drugs than their peers who are supported (HRC Foundation).
Lack of safe spaces: Bars and clubs often become the only safe or affirming environments, inadvertently normalizing substance use as a part of community belonging.
Barriers to care: Many LGBTQ+ individuals avoid seeking treatment out of fear of being judged, misgendered, or misunderstood by providers unfamiliar with their unique needs.
